Turbine engines are extremely reliable and many business jet pilots go through their entire careers experiencing engine failures only during simulator training. But in mid-July, two Beechjet 400A pilots found out what it’s like to lose not just one engine in flight, but both of them. Fortunately for them and their seven passengers they were able to get one of the light jet’s engines restarted during the descent.

The pilot and his passenger escaped with minor injuries after their King Air A90 was severely damaged during a crash landing at Ferguson Airport, Pensacola, Fla., on August 17. The turboprop twin, N45TT, was taking off for a post-maintenance test flight when it turned to return to the airport. VMC prevailed.
The UK’s August 20 final report of the fatal crash of a Challenger 604 at Birmingham International Airport, England, on Jan. 4, 2002, concluded that the crew failed to remove frost from the wings and might have been impaired by the combined effects of non-prescription sleep aids, jet lag and fatigue. Both pilots and three passengers were killed after the airplane crashed and burst into flames on the runway.
A Sabreliner owned and operated by Compass Acquisitions and Development in Dallas ran off the runway at Brownwood Regional Airport, Texas, on May 9 following an engine failure at V1. The pilot set the nosegear back on the runway but was unable to stop the aircraft before it went off the end of the runway. The airplane traveled another 1,300 feet before finally stopping, according to the NTSB.
The FAA has issued a special airworthiness information bulletin to alert operators of Bombardier CL600-2B19 regional jets of “higher-than-expected levels of dual-channel disconnects of the horizontal stabilizer trim system.” While Bombardier is undertaking an engineering investigation to address the cause of this problem, the OEM has issued a service letter intended to help reduce the number of events.
Gerry Garapich and fellow passenger Bob Powelson are recovering from the injuries they sustained on May 5 during a rough landing at North Las Vegas Airport. Garapich had to take over flying a Twin Commander turboprop after their friend (and the airplane’s pilot) suffered an apparent heart attack (and later died).
NTSB public hearings are scheduled to take place June 13 to 15 on the crash of a Pinnacle Airlines Bombardier CL-600 regional jet in Jefferson City, Mo., on October 14 last year. The two crewmembers were killed. There were no passengers on board and no injuries on the ground. The flight was repositioning from Little Rock, Ark., to Minneapolis-St. Paul when it lost power in both engines at FL410.
Bombardier CL-600-2B19 CRJ, West Grove, Pa., Oct. 17, 2005–The NTSB blamed the accident on the separation of the exhaust nozzle due to inadequately designed attachment hardware.
Dassault Falcon 20D, Lorain, Ohio, Sept. 1, 2005–The NTSB said the USA Jet Airlines Falcon accident was caused by the “ingestion of multiple birds in each engine at takeoff, which resulted in a complete loss of engine power.” Falcon 821AA was taking off from Lorain County Regional Airport when a flock of birds flew into both engines.
Swearingen SA-226TC, Denver, April 5, 2006–The NTSB blamed the accident on the failure of the elevator down cable due to improper routing. A factor was the improper inspection of the elevator cable by maintenance personnel. The installation caused the cable to wear against a guide until it failed.
