FOKKER F28-100, Manchester, UK, APRIL 6, 2002–A misunderstanding of the flight manual resulted in damage to the horizontal stabilizer tip and leading edge of a Fokker F28 Mark 0100 (G-UKFI) at Manchester Airport. There were no injuries to those aboard, including the ATP-rated captain and his copilot.
CESSNA 441, MEDFORD, ORE., JUNE 18, 2002–The pilot and three passengers on board a Cessna 441 N564AC were uninjured when the airplane’s right main gear collapsed while on landing roll at Rogue Valley International Airport (MFR). The accident occurred at 7:50 p.m. while operating under Part 91. N564AC, operated by Erickson Air-Crane, sustained substantial damage.
RAYTHEON BEECH KING AIR 90, ROSEAU, MINN., JUNE 27, 2002–Runway incursions took on a new meaning when the two pilots and two medical personnel on board King Air N911ND escaped injury during a 4:36 a.m. accident at Roseau Municipal Airport (ROX).
RAYTHEON BEECH KING AIR 300, NEAR TULSA, OKLA., JUNE 22, 2002–There were no reported injuries as a result of N224CR’s in-flight separation of a portion of the left-engine cowling during cruise flight. The twin turboprop was owned by Ponca City Transportation of Ponca City, Okla., and operated by Flight Concepts of Tulsa, Okla., under Part 91.
BELL 47G4, NEAR GRAND LAKE, LA., JUNE 23, 2002–The pilot of the Bell 47G4 and one passenger received minor injuries while a second passenger escaped unharmed when the aircraft (N62446) hit terrain while maneuvering near Grand Lake at 11:55 a.m. The helicopter, registered to and operated by Deep South Helicopters of Jennings, La., was being operated under Part 91, and no flight plan was filed.
ENSTROM F-28C, NEAR MEDICINE BOW, WYO., JUNE 10, 2002–N5694Z, operated by Falcon Helicopter and piloted by a commercial-rated pilot, was substantially damaged during a hard landing at 9:45 a.m. near Medicine Bow. The pilot sustained serious injuries, and the passenger received only minor injuries. The weather was reported to be VMC for the flight, which originated in Laramie, Wyo., at 7:30 a.m.
BELL 206BII, NEAR NHULUNBUY, AUSTRALIA, JUNE 5, 2002–About 4 p.m. Bell 206BII VH-PHA, operated by Laynhapuy Homelands Association of Nhulunbuy (Gove), Northern Territories, crashed while on an aerial survey mission about 87 nm southwest of Nhulunbuy. Four of the five people on board were killed, and the helicopter was destroyed.
DORNIER 328, WASHINGTON, D.C., JUNE 17, 2002–Failure to follow operational procedures resulted in a serious injury to a U.S. Airways Express flight attendant during a scheduled Part 121 flight from Ronald Reagan Washington National Airport (DCA).
MBB BO-105S, SPARTA, ILL., JUNE 8, 2002–N660SL, operated by Arch Air Medical, sustained substantial damage when a static line broke loose and hit the main rotor while hot refueling after landing at Hunter Field Airport (SAR).
