Loss of engine power during landing approach due to a fatigue fracture in a power turbine blade caused the fatal helicopter crash, according to the Board. N67GE, operated by Island Express Helicopters, was landing on Santa Catalina Island at the end of a Part 135 flight from Long Beach when witnesses heard a “pop” sound and saw flames emanating from the back of the engine. The helicopter then crashed and was consumed by fire.

The CitationAir Sovereign was substantially damaged after an encounter with turbulence during cruise flight. The twinjet was on a Part 91 positioning flight in clear air from Denver’s Centennial Airport to Eagle County Airport when it was buffeted by extreme turbulence that lasted less than a minute. As the autopilot disengaged, the crew was briefly unable to keep the wings level and maintain altitude or airspeed.
The pilot made an emergency landing after the turboprop single suffered engine and instrument failure during a Part 91 flight from San Luis Obispo Airport to Fresno-Yosemite Airport. After the power-off touchdown at Lost Hills-Kern County Airport, the TBM overran the runway and shed its nosegear. The aircraft sustained substantial damage to its wings and fuselage. The owner-pilot and four passengers were uninjured.
At the conclusion of an uneventful Part 121 flight from San Francisco to Ontario, the crew attempted to lower the landing gear. Only the main gear extended and the crew used emergency procedures in an unsuccessful attempt to extend the nosegear. Following an aborted landing, a low-level flyby past the tower confirmed that the nosegear doors were open but the nosegear was still in its retracted position.
The helicopter was destroyed and the commercial pilot and one passenger killed when it crashed following inadvertent flight into IMC. The other passenger was seriously injured.
After lifting off into clouds following a rescue operation, the helicopter struck something on the mountainside, and then landed on a steep ridgeline before rolling downhill approximately 800 feet.
Cirrus Aircraft’s SF50 Vision single-engine jet program is “making good progress,” according to president and CEO Brent Wouters, who added that the company has spent $64 million so far on the program and will need to spend another $64 million to see the aircraft through certification. The order book as of early June stood at 431 aircraft, 106 of which were written in the first half of the year.

Bombardier BD-100-1A10, Wheeling, Ill., Feb. 24, 2010–The Challenger 300 was substantially damaged when it was struck by a de-icing vehicle while preparing for departure for a Part 91 flight from Chicago Executive Airport. Marginal VMC existed at the time of the accident. Neither pilot on board was injured.
GE Aviation delivered the 5,000th CF34 turbofan engine. According to the engine manufacturer, the CF34 has achieved a dispatch reliability rate of 99.95 percent over more than 50 million flight hours. The next-generation CF34, which will include GE’s eCore technologies, is in development and could enter service by 2015.
