The Italian aviation safety agency, ANSV, issued four recommendations on August 2 to the EASA and Transport Canada about the Pratt & Whitney Canada PW127 turboprop engine that powers the ATR 72 regional airliner. The recommendations came after a series of engine failures that all occurred during the initial climb.

BOMBARDIER LEARJET 45, MILAN, ITALY, JUNE 1, 2003–At approximately 3:26 p.m., Learjet 45 (I-ERJC), operated by Linate-based air-taxi operator Eurojet Italia, crashed after takeoff from Milan Linate Airport. The airplane reportedly struck a flock of birds shortly after takeoff, and airport officials later found the charred remains of six birds on the runway.

Accident investigators have determined that Tunisian mechanics replaced a faulty fuel gauge in the ATR 72 that crashed off the northeast coast of Sicily on August 6 with the wrong model, a mistake that apparently led the doomed airplane’s pilots to upload less fuel than they needed to complete their trip from Bari, Italy, to Djerba, Tunisia.
