The FAA is adopting a new Airworthiness Directive (FAA-2011-0518) for Airbus A300/310 airliners to prevent high loading of the vertical stabilizer caused by excessive rudder pedal inputs, which could cause failure of the vertical stabilizer and consequent loss of control. The AD, effective Dec. 14, 2012, applies to A300 B4-600, B4-600R, F4-600R and C4-605R Variant F airplanes (collectively called the A300-600 series), as well as to the A310 series.
American Airlines Flight 587
Steven Chealander, the public face of the NTSB during press conferences following the February 12 crash of Continental Connection Flight 3407 on approach to Buffalo Niagara International Airport, has left the safety agency and joined Airbus Americas.
A FedEx MD-11 cargo aircraft crashed upon landing at Tokyo Narita International Airport early this morning, bursting into flames and killing the two American pilots. The flight originated in Guangzhou, China, and crashed at 6:49 a.m. local time in high winds. The airplane came to rest on its back after it hit the runway, bounded upward, turned over and slid several yards.
You said recently that the NTSB could become more proactive if it investigated incidents, as well as accidents. Please explain.
As an outgrowth of its continuing investigation into the November 12 crash of American Airlines Flight 587 in New York City, the NTSB has recommended that the FAA order manufacturers and operators of transport-category airplanes to revamp pilot-training programs with regard to rudder use.
Former National Highway Traffic Safety Administration (NHTSA) boss Marion Blakey became the ninth chairperson of the National Transportation Safety Board (NTSB), taking over from Jim Hall in late September. Hall resigned last January after seven years with the independent safety agency, six of them as chairman.
Menzies Aviation Group employees Dennis Blair and Sylvie Greleau were two of the casualties on American Airlines Flight 587, the Airbus A300 that crashed in Queens, N.Y., last month. Blair, 55, was Menzies’ managing director and senior vice president for the Americas. Greleau, 40, was director of sales and marketing for the Americas. Greleau worked for Ogden Ground Services, before its sale to Menzies Aviation Group in November last year.
Marion Blakey took over the National Transportation Safety Board as its ninth chairman just in time to enter the maelstrom surrounding last November’s crash of American Airlines Flight 587 in New York. Through frequent media briefings in the days following the accident, she helped allay the fears of an American public–still jittery over air travel–that the crash was caused by another terrorist attack.
The fatal crash of an Aerospatiale AS 350 at the Grand Canyon on August 10 was the fourth fatal accident of a Papillon Helicopters tour flight since September 1985 and the second for the operator in the Grand Canyon since April 1999 (in which the sole pilot was the only one killed).
Investigations of aircraft accidents–referred to in the past as “kicking tin”–are taking a decidedly high-tech turn, and NTSB chairman Marion Blakey predicted that crash probes are going to be driven increasingly by issues involving high-tech safety systems, integrated computer programs, high-grade materials and electronically generated data and data analysis.
