An easy day of flying is not hard to define. Passengers arrive on time, good weather translates into few delays and everything on the airplane works the way it was intended. Identifying a difficult day is a bit more challenging. Is it when the crew shoots a localizer approach to minimums at night with thunderstorms all around?

This summer a Eurocopter EC 155 all-weather demonstrator performed satellite-guided precision approach tests in Lausanne, Switzerland, as part of a research program aimed at developing approach and departure IFR procedures suited to helicopters using the European geostationary navigation overlay service (egnos) Skyguide, the Swiss air navigation service provider, coordinated the program. Swiss medical air rescue agency Rega was also involved.

Swearingen SA-26AT, Jacksonville, Fla., Nov. 27, 2003 – The NTSB concluded that the commercial pilot of the SA-26 that crashed on an ILS approach to Craig Airport descended below decision height in low ceilings and fog. A factor was the pilot’s decision to attempt the instrument approach with weather below the prescribed minimums.

The FAA published a broad final rule last month that incorporates Rnav definitions, clarifies the intent of obstacle departure procedures and modifies definitions to seek consistency with ICAO wording. With this rule, the FAA is amending FAR Part 1.1 to include or amend definitions for decision height, decision altitude and final approach fix.

Beech C99, Butte, Mont., March 18, 2006–The NTSB attributed the Ameriflight cargo airplane’s crash into mountains to the copilot’s failure to follow the published instrument approach procedure and the captain/PIC’s inadequate supervision. Factors in the accident were snow and mountain obscuration. In addition, IMC in the area included icing conditions and poor visibility.

New regulations being developed by Europe’s Joint Aviation Authorities (JAA) may soon enable operators of HUD-equipped aircraft to hand-fly Cat II and Cat IIIA precision approaches and conduct Cat I approaches with a reduced runway visual range (RVR) of 400 meters instead of 550.

Pilatus PC-12, Bellefonte, Pa., March 26, 2005–The NTSB determined the probable cause of the crash of the PC-12 on an ILS approach to University Park Airport, State College, Pa., was the pilot’s failure to maintain sufficient airspeed to avoid a stall, which resulted in an inadvertent stall and spin. Factors also included the pilot’s failure to follow procedures/directives and clouds.
