Piaggio Avanti, Fort Lauderdale, Fla., March 20, 2007–The left landing gear of the Avantair Avanti collapsed on landing at Hollywood International Airport. The left tire blew out and separated from its rim. No one was injured but the airplane was substantially damaged.

Cessna Citation 560XL, Wheeling, Ill., Nov. 11, 2003–Citation N789CN was substantially damaged when it overran the end of the runway during an aborted takeoff at Palwaukee Municipal Airport (PWK).

LEARJET 25, ITHACA, N.Y., AUG. 24, 2001–Investigators are delving into the circumstances that caused a Learjet to crash into a fence on takeoff from the Tompkins County Airport in Ithaca. The flight lasted only seconds and the business jet came to rest less than half a mile from the departure end of Runway 32. Both of the ATP-rated pilots perished.

The NTSB has asked the FAA to “explicitly prohibit” position and hold clearances at the intersections of active runways during low-visibility conditions and at night. While the recommendation is intended for all airports that have such a runway configuration, the Safety Board’s request stems from a loosely related incident on Jan. 25, 2002, at Ted Stevens Anchorage International Airport in Alaska.

BEECHJET 400A, ST. AUGUSTINE, FLA., AUG. 18, 2003–Beechjet N51NP, registered to AC Expeditions and operated by Executive Beechcraft, lost directional control on takeoff from St. Augustine Airport (SGJ) at 7:18 a.m. EDT. The aircraft was substantially damaged, but neither the ATP-rated pilot nor copilot was injured. There were no other occupants in the aircraft, which was operating in VMC on an IFR flight plan.

On May 8 a Beechjet 400 (N400GJ) operated by Georgia Jet was substantially damaged when it struck birds while departing from Runway 6L at Cleveland Burke Lakefront Airport. The crew did not see any birds on or around the runway as the jet began its takeoff roll. When the aircraft reached V1, birds hit the airplane and the pilot aborted the takeoff, bringing the Beechjet to a stop about 100 ft before the end of the runway.

A cargo-configured Falcon 20 made a gear-up landing at Detroit City Airport shortly after takeoff on August 28, sliding off the end of the runway and coming to rest among the headstones of a cemetery adjacent to the airport on the city’s east side.

There was no indication that the internal control lock was engaged on a Citation 550 that careened off the runway during its takeoff roll at Wiley Post Airport, in Bethany, Okla., last month, according to NTSB’s preliminary investigation. The pilot claims that when reaching V1 (103 kt), the nosewheel did not lift off the runway when he began to pull back on the control column.

Airbus A300-600F, El Paso, Texas, June 10, 2002–The NTSB determined that the cause of the tail strike the FedEx Airbus suffered on takeoff from El Paso International was the flight crew’s failure to obtain Vr speed before rotation, which resulted in insufficient lift. The crew had entered the V-speeds in the flight-management system (FMS) and both crewmembers’ primary flight displays (PFDs) showed no discrepancies.

Cessna 525A CitationJet, Dexter, Maine, Oct. 7, 2002–Citation N57EJ was substantially damaged during a runway overrun at Dexter Regional Airport (1B0). The ATP pilot and one passenger received serious injuries, and two passengers received minor injuries.
