An EC 130 operated by New York City’s Liberty Helicopters made a forced landing into the Hudson River last month during an otherwise routine sightseeing flight. According to the NTSB’s preliminary report, the pilot, who was flying at between 300 and 400 feet, reported hearing a loud bang, saw debris fly by her windshield and then lost all yaw control.

Meggitt Avionics is displaying its secondary flight display system, secondary flight display repeater and magnetometer here at its stand (Hall 2B Stand L13A). The company reported it has recently received a $20 million, nine-year contract to supply the system, including spares and support, for the Apache Longbow.
The pilot’s decision to rapidly maneuver the helicopter at a high density altitude near steeply sloping terrain was the cause of a fatal air-tour helicopter crash on Aug. 10, 2001, according to the NTSB’s final report. The pilot and five passengers were killed and one passenger was seriously injured when the Papillon Airways AS 350 hit terrain during an uncontrolled descent near Meadview, Ariz.
BEECH 65-A90, FENTRESS, TEXAS, OCT. 17, 2003–King Air N511BF was destroyed during a forced landing at approximately 3:30 p.m., following a loss of engine power near Fentress. The sole-occupant commercial-rated pilot sustained minor injuries. The King Air A90 was registered to American Aircraft Sales International of Venice, Fla., and was operated by Sky Dive San Marcos of San Marcos, Texas.
EUROCOPTER EC 120B, JONESBORO, GA., JAN. 24, 2004–At 4:50 p.m. Eurocopter N125MG hit the ground during a practice autorotation at the Hampton/Clayton County/Tara Airport (4A7) near Jonesboro. The VFR instructional flight was operated under Part 91 and no flight plan was filed for the local flight.
Pilots know that the primary job of EGPWS is to keep them from running into the ground, but they might not be aware that it can also warn them if they are about to land on the wrong runway, take off from a taxiway or cross an active runway, among a host of other incursion-preventing functions.
New taxi into position and hold (TIPH) guidance for pilots became effective February 5. It includes new ATC procedures and phraseology to improve runway safety.
Because of “undesirable” events involving TIPH, the FAA convened a safety risk management panel consisting of representatives from the agency’s Air Traffic Service and Flight Standards Service, as well as certain specialists, including experts in aviation human factors.
Following last year’s NTSB recommendation that turbine-powered helicopters carry terrain awareness and warning systems (TAWS), Air Logistics has decided to upgrade its newer medium and heavy twin-engine helicopters operating in the Gulf of Mexico with Honeywell’s enhanced ground proximity warning system (EGPWS).
The FAA has certified the CMS-1000 communication management system from Pentar Avionics. The panel-mount unit provides digital and analog input and output, ACARS processing, an optional internal VDL Mode 2 (VHF datalink) radio and optional GPS sensor.
