Italian accident investigators have concluded that human error on the part of two German pilots of a Cessna Citation CJ2 was the main cause of a fatal collision with a McDonnell Douglas MD-87 at Milan Linate Airport on Oct. 8, 2001.

The final report of the Oct. 8, 2001 collision between a Citation II and a Scandinavian Airline System MD-87 at Italy’s Milan-Linate Airport said the immediate cause of the crash, which killed 114 people aboard both jets and four ground workers, was that the Citation inadvertently entered the active runway as the airliner was reaching V1 on its takeoff roll.
As Comair Flight 5191 accelerated down an unlit runway into the predawn darkness at Blue Grass Airport in Lexington, Ky., last August 27, the captain–the nonflying pilot–called out “V one, rotate” followed by “whoa” and then an expletive.
In the June 2005 issue of AIN I wrote about a fully loaded British Airways 747 that was taking off from Los Angeles bound for London when one of its engines emitted a large fireball just after liftoff. I commented on the wisdom (or lack of wisdom) of proceeding on a 16-hour flight with one engine out and uncertainty about the extent of damage to the engine or the airframe.
