As a result of the Comair Flight 5191 crash in Lexington, Ky., the NTSB issued five safety recommendations to the FAA this week in an effort to mitigate airport surface operation errors. The Safety Board recommends that the FAA require Part 91K, 121 and 135 operators to establish procedures requiring crewmembers to confirm and cross-check the airplane’s location before crossing the hold-short line for takeoff; require operators to install cockpit moving-map displays or an automatic system to alert crewmembers of an attempted takeoff on a runway other than the one intended; and require Part 139 (airport) operators to implement enhanced taxiway centerline markings and surface holding position markers. The Safety Board also recommends that the FAA prohibit the issuance of a takeoff clearance until the aircraft has crossed all intersecting runways and revise FAA Order 7110.65 to indicate that controllers should not perform administrative tasks when moving aircraft are in the controller’s area of responsibility.
